• 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 110/70, P71.

• 4:45pm Diaper is dry.

• 4:50pm Dinner: Lamb shank with gravy , mashed potato, asparagus and carrot, and 3/4 cup of water. Coughed once.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a medium wet diaper with a small-formed BM, applied Cavilon barrier cream. Wipe/dry inner thighs fading redness with a damp cloth. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• 6:50pm Positioned on her left side inserted, Maureen goes to sleep for the night.
